About The Position

This full-time position is for a Home Health RN Case Manager serving the South Lake county area, including Palatine, Kildeer, Long Grove, Deer Park, and surrounding areas. The role involves developing and maintaining therapeutic relationships, providing individualized care, and collaborating with patients and families throughout the healthcare continuum. The position requires assessing patient needs, planning and implementing care, and mobilizing appropriate resources to ensure quality, cost-effective care. The RN will also participate in quality improvement initiatives, adapt practice to evidence-based literature, and ensure regulatory compliance. Education and development of patients, families, peers, and colleagues are key aspects of the role. The Nurse/Patient Relationship involves adaptive communication, promoting comfort, and involving patients/families in care planning. Assuring/Improving Quality Care focuses on facilitating patient rights, mobilizing resources, adapting to standards, participating in quality improvement, reducing risk, and meeting regulatory requirements. Patient/Staff Education emphasizes meeting learning needs, utilizing resources, creating educational environments, and assessing readiness to learn. The Nursing Process is used as a problem-solving model, involving recognition of clinical changes, evaluation of care plans, sound clinical decision-making for complex patients, prioritization, and delegation. Initiating referrals, discharge planning, and ethical decision-making are also crucial. Leadership/Collaboration/Professional Development involves respecting peers and disciplines, participating in the care team, demonstrating leadership, developing self and others, participating in unit activities, maintaining knowledge, and demonstrating effective communication and conflict resolution skills. The role requires critiquing one's own practice and achieving required certifications or a Bachelor's Degree, along with earning a minimum of 10 contact hours per year.

Requirements

  • Graduate from an accredited school of nursing
  • One year of medical surgical experience
  • Has as demonstrated proficiency for the requirements for RN role
  • Good communication and interpersonal skills
  • Good documentation skills.
  • Computer skills
  • Good time management and organizational skills
  • Familiarity with patient care equipment
  • Current Illinois RN license
  • Current CPR
  • A valid driver’s license issued by the Division of Motor Vehicles.
  • Must be able to travel to all areas within the Advocate service area in all extremes of weather on all types of roads.
  • Must be willing to work occasional overtime if needed.
  • Must be able to assist with ambulation including turning and transferring patients up to 200 pounds.
  • Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
