About The Position

This full-time position for a Home Health RN Case Manager serves the South Lake county area, including Palatine, Kildeer, Long Grove, Deer Park, and surrounding areas. The role involves developing and maintaining therapeutic relationships, providing individualized care, and collaborating with patients and families throughout the healthcare continuum. The position requires assessing patient needs, planning and implementing care, and mobilizing appropriate resources to ensure quality, cost-effective care. The RN will adapt practice to evidence-based standards, participate in quality improvement initiatives, and meet regulatory requirements. Education and support for nursing peers and colleagues are also key aspects of the role. The position utilizes the nursing process as a problem-solving model, requiring recognition of clinical changes, sound clinical decision-making for complex patients, and effective prioritization and organization of care. Initiation of referrals, discharge planning, and coordination through an interdisciplinary approach are essential. Ethical decision-making, monitoring for confidentiality, and correct delegation to unlicensed assistive personnel are also expected. Leadership behaviors, collaboration with peers and other clinical disciplines, and commitment to professional development are crucial. The RN is expected to maintain current knowledge, demonstrate effective communication, and critique their own practice.

Requirements

• Graduate from an accredited school of nursing
• Earn 10 continuing education credits annually
• One year of medical surgical experience
• Has as demonstrated proficiency for the requirements for RN role
• Good communication and interpersonal skills
• Good documentation skills.
• Computer skills
• Good time management and organizational skills
• Familiarity with patient care equipment
• Current Illinois RN license
• Current CPR
• Ability to work weekends, holidays, and different shifts in order to accommodate staffing
• Ability to take “on call” assignments.
• Ability to handle multiple tasks and complete work within short timeframes.
• Ability to transport objects, patients, and equipment
• Ability to respond quickly to patient needs and work at fast pace
• Ability to respond to patients in a timely manner and access all areas of the unit
• Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
• Must be able to travel to all areas within the Advocate service area in all extremes of weather on all types of roads.
• Must be willing to work occasional overtime if needed.
• Must be able to assist with ambulation including turning and transferring patients up to 200 pounds
