The Home Health RN Case Manager develops and maintains a therapeutic relationship throughout the healthcare continuum. This role provides individualized care to promote physical and psychosocial comfort, assesses patient and family readiness, and involves them in planning and implementing care. The position mobilizes appropriate resources, adapts practice to the latest standards, and participates in quality improvement initiatives. The RN Case Manager also demonstrates a commitment to meeting the learning needs of patients and families, assists in creating an environment that promotes educational growth for peers, and assesses patient and family readiness to learn. They demonstrate the use of the nursing process as a problem-solving model, recognize changes in clinical situations, and evaluate and implement changes to the plan of care. The role involves initiating referrals and discharge planning, applying ethical decision-making, monitoring for confidentiality, and delegating correctly to unlicensed assistive personnel. Additionally, the RN Case Manager demonstrates respect and understanding for peers and other clinical disciplines, participates as an effective member of the patient care team, and demonstrates leadership behaviors. They are committed to the development of self and others, participate in unit meetings, and utilize available resources to maintain a current knowledge base. The role requires effective communication, feedback, and conflict resolution skills, as well as the ability to critique their own nursing practice.
