About The Position
Concierge Home Care is hiring a full-time Home Health Registered Nurse to serve the Southside and Beaches area of Jacksonville, Florida. The role offers a pay-per-visit compensation model, with average annual earnings of $80,000–$85,000+ and higher potential with weekend-focused scheduling. Patient volume is provided, and territories are designed to minimize drive time through clustered scheduling. The position offers control over daily scheduling and caseload, autonomous 1:1 patient care, and a flexible work week without traditional hospital schedules or 12-hour shifts. Nurses participate in a required on-call rotation. Technology includes AI-assisted charting and the WellSky/Kinnser EMR to streamline documentation. The organization provides structured onboarding, mentorship, preceptorship, ongoing training, and advancement opportunities. Employment is contingent upon successfully completing an AHCA Level 2 background screening.
Requirements
- Hold an active, unencumbered Florida Registered Nurse (RN) license.
- Have a valid driver’s license, reliable transportation, and auto insurance.
- Successfully complete an AHCA Level 2 background screening as a condition of employment.

- Home health experience is preferred but not required.
Responsibilities
- Perform comprehensive patient assessments in the home setting.
- Develop and manage individualized care plans.
- Provide skilled nursing care and educate patients and caregivers.
- Administer treatments and monitor patient condition.
- Coordinate care with physicians and interdisciplinary teams.
- Evaluate patient progress and adjust care as needed.
- Complete accurate, timely documentation using streamlined tools.
- Build and manage a daily schedule and caseload.
- Participate in the required on-call rotation.
