Requirements

• Graduate of an accredited school of nursing.
• One (1) to two (2) years of recent acute care experience in an institutional setting.
• Current licensure in state and CPR certification.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
• This position requires regular travel by personal vehicle between patient homes, the office, and other locations throughout the assigned service area.
• The clinician must hold a valid driver's license, maintain reliable transportation and current auto insurance, and be able to drive safely for extended periods in varying weather and traffic conditions.
• Pre-employment physical screening, or a letter from a recent healthcare encounter confirming the candidate is free of communicable disease
• Documented negative TB test (TST or TB blood test) taken within the past 12 months
• TB risk assessment and symptom evaluation upon hire

Responsibilities

• Assess each new patient's condition, history, and home care needs
• Build the care plan with the patient and family
• Update the care plan as the patient's status changes
• Administer medications and treatments as prescribed
• Teach patients and families how to manage care at home
• Coordinate with physicians and send updates at least every 60 days
• Coordinate with community health partners involved in the patient's care
• Plan and carry out discharge when the patient is ready
• Arrange any equipment or services the patient needs
• Check in on home health aide care every two weeks
