Home Health Physical Therapist-Per Diem
About The Position
CenterWell Home Health is hiring a per diem Home Health Physical Therapist who reports to the Clinical Manager. The therapist provides personalized physical therapy to patients in their homes, helping them regain strength, mobility, independence, and functional ability. The role includes assessing patients, developing and delivering rehabilitation programs consistent with physician treatment plans, and coordinating care with patients, families, physicians, and other members of the care team. The position is patient-facing and requires TB screening. It also participates in Humana's driver safety program, requiring a valid state driver's license and maintained personal vehicle liability insurance.
Requirements
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid state driver's license.
- Maintain personal vehicle liability insurance at the minimum required limits for the state of residence, or $25,000 bodily injury per person, $25,000 bodily injury per event, and $10,000 property damage, whichever is higher.
- Complete TB screening if selected for the role.

Responsibilities
- Plan and administer prescribed physical therapy treatment to patients in their homes to help them attain the highest level of physical function.
- Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement rehabilitation programs consistent with the physician's Plan of Treatment and the patient's goals.
- Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
- Promptly and thoroughly document patient observations, interventions, and evaluations.
- Report patient progress to the physician, Clinical Manager, care team, patient, and family.
- Submit required evaluations, plans, progress notes, and discharge summaries.
