About The Position
As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility, and independence in their homes. You will deliver personalized care focused on functional improvement and help patients overcome limitations and improve their quality of life. The role involves planning and administering prescribed physical therapy, assessing patient strengths, developing and implementing rehabilitation programs, overseeing PTAs and Home Health Aides, documenting care, and communicating progress to the care team and others involved in the patient's care. This is a per-diem, patient-facing role. It requires TB screening, a valid state driver's license, and personal vehicle liability insurance meeting the stated minimum limits. The position is scheduled for 1 hour per week. Pay is listed as a per-visit/unit rate of $65.00–$91.00. The posting also provides a full-time annual base-pay estimate of $102,300–$140,800, which may vary by location and individual qualifications.
Requirements
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid state driver's license.
- Maintain personal vehicle liability insurance in accordance with state minimum limits or the stated limits, whichever is higher.
- Complete TB screening if selected for the role.

Responsibilities
- Plan and administer prescribed physical therapy treatment for patients in their homes to help them attain the highest possible level of physical function.
- Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement rehabilitation programs consistent with the physician's Plan of Treatment and the goals of the patient team.
- Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
- Promptly and thoroughly document patient observations, interventions, and evaluations.
- Report patient progress to the physician, Clinical Manager, care team, patient, and family.
- Submit required evaluations, plans, progress notes, and discharge summaries.
