About The Position

CenterWell Home Health is hiring a per-diem Home Health Physical Therapist for the Ft. Myers, Estero, and San Carlos Park areas. Reporting to the Clinical Manager, the therapist provides personalized physical therapy in patients’ homes to help them regain strength, mobility, independence, and physical function. The role includes assessing patients, developing and administering rehabilitation plans consistent with physician instructions, coordinating care with the patient’s care team, and documenting and reporting treatment and progress.

Requirements

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid state driver’s license.
• Maintain personal vehicle liability insurance as required by the role and applicable state requirements.

Nice To Haves

• One year of experience as a Physical Therapist.

Responsibilities

• Plan and administer prescribed physical therapy treatments in patients’ homes to help them attain the highest possible level of physical function.
• Assess and document patients’ physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement rehabilitation programs consistent with the physician’s Plan of Treatment and the goals of the patient team.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Promptly and thoroughly document patient observations, interventions, and evaluations.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family.
• Submit required evaluations, plans, progress notes, and discharge summaries.
