About The Position
Always Best Care is a Medicare-certified home health agency committed to helping patients remain safe, independent, and active in their own homes. Our therapy team plays an essential role in improving mobility, strength, and overall quality of life for the seniors we serve. We are currently seeking licensed Physical Therapists to provide home health services across Oʻahu.
Requirements
- Current Hawaii Physical Therapist (PT) license
- Minimum 1 year clinical experience
- Strong documentation and communication skills
- Ability to work independently in a home setting
- Valid driver’s license and reliable transportation
Nice To Haves
- home health experience preferred
Responsibilities
- Provide skilled physical therapy visits in patients’ homes
- Conduct PT evaluations and follow-up assessments
- Develop individualized treatment plans focused on mobility, strength, balance, and fall prevention
- Educate patients and family caregivers on safe mobility techniques
- Maintain accurate, Medicare-compliant documentation
- Collaborate with the care team to support patient progress
