About The Position

Elara Caring is seeking a PRN Home Health Physical Therapist to serve patients in the La Grange area within the Bryan College Station territory. The therapist provides individualized physical therapy in patients’ homes, connecting treatment to patients’ everyday environments and personal goals. The position calls for consistent availability, including communicating the days and hours the therapist can work each week. The therapist works with patients one-on-one and collaborates with physicians, caregivers, and an interdisciplinary care team to support recovery, mobility, strength, balance, and independence.

Requirements

• Graduate from an APTA-approved physical therapy program.
• Hold a current, unrestricted Texas physical therapist license.
• Have at least one year of experience in a healthcare facility.
• Have a valid driver’s license and acceptable driving record, or reliable transportation between patient homes and work locations.
• Be able to frequently lift, push, pull, and support up to 50 pounds, including for patient positioning, transfers, and moving equipment.
• Commit to consistent availability and identify the days and hours available to work each week.

Responsibilities

• Evaluate patients’ functional needs and create individualized treatment plans.
• Provide skilled physical therapy in patients’ homes.
• Track patient progress and adjust care as needs change.
• Teach patients and caregivers strategies to support recovery between visits.
• Coordinate care with physicians and other healthcare professionals.
• Complete timely and accurate clinical documentation.
• Supervise therapy assistants and support team members as appropriate.
