About The Position
Advantage Home Health is seeking a compassionate and dedicated Physical Therapist to provide home health care in Carlisle, Pennsylvania. In this PRN role, the therapist manages their schedule and provides one-on-one care in patients’ homes, helping them regain strength, mobility, and independence. The position offers flexible scheduling, primarily daylight hours, and support from clinical and leadership teams.
Requirements
- Graduate of an accredited Physical Therapy program.
- Current Physical Therapist license in Pennsylvania.
- Current CPR certification or ability to obtain it upon hire.
- Valid driver's license, reliable transportation, and proof of auto insurance.
- Strong communication, organizational, and documentation skills.
Nice To Haves
- Home health experience is preferred but not required.
Responsibilities
- Provide skilled physical therapy services in accordance with physician orders and individualized plans of care.
- Evaluate patients and develop personalized treatment programs to improve function and quality of life.
- Assess mobility, strength, balance, and safety needs.
- Document patient evaluations, treatments, and progress in a timely and accurate manner.
- Collaborate with physicians, nurses, and other healthcare professionals to support optimal patient outcomes.
- Educate patients and caregivers on exercises, safety techniques, and ongoing therapy goals.
- Supervise Physical Therapist Assistants as appropriate.
