About The Position
Advantage Home Health is seeking a compassionate and dedicated Physical Therapist to provide home health care in Altoona, Pennsylvania. The PRN role offers schedule flexibility and primarily daylight hours. The therapist manages their schedule and provides one-on-one care in patients’ homes, helping them regain strength, mobility, and independence. The position includes providing services under physician orders and individualized plans of care, documenting patient progress, and collaborating with other healthcare professionals. Home health experience is preferred but not required.
Requirements
- Graduate of an accredited Physical Therapy program.
- Current Physical Therapist license in Pennsylvania.
- Current CPR certification or ability to obtain it upon hire.
- Valid driver's license, reliable transportation, and proof of auto insurance.
- Strong communication, organizational, and documentation skills.
Nice To Haves
- Home health experience is preferred but not required.
Responsibilities
- Provide skilled physical therapy services in accordance with physician orders and individualized plans of care.
- Evaluate patients and develop personalized treatment programs to improve function and quality of life.
- Assess mobility, strength, balance, and safety needs.
- Document patient evaluations, treatments, and progress in a timely and accurate manner.
- Collaborate with physicians, nurses, and other healthcare professionals to support optimal patient outcomes.
- Educate patients and caregivers on exercises, safety techniques, and ongoing therapy goals.
- Supervise Physical Therapist Assistants as appropriate.
