Home Health Physical Therapist - PRN
About The Position
As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility, and independence in their homes. You will provide personalized care focused on functional improvement, helping patients overcome limitations and improve their quality of life. The role involves planning and administering physical therapy, assessing patients, developing rehabilitation programs, overseeing support staff, documenting care, and reporting progress to the physician and care team. This is a patient-facing role that requires TB screening and participation in Humana's driver safety program. The position is PRN, with 1 scheduled weekly hour.
Requirements
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid state driver's license.
- Maintain personal vehicle liability insurance meeting the applicable state minimum limits or the stated higher limits.

Responsibilities
- Plan and administer prescribed physical therapy treatment for patients in their homes to help them attain the highest possible level of physical function.
- Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement a rehabilitation program consistent with the physician's Plan of Treatment and the goals of the patient team.
- Provide PTAs and Home Health Aides with clear written care instructions, and oversee their performance and documentation.
- Promptly and thoroughly document patient observations, interventions, and evaluations.
- Report patient progress to the physician, Clinical Manager, care team, patient, and family.
- Submit required evaluations, plans, progress notes, and discharge summaries.
