PRN Home Health Physical Therapist
About The Position
Elara Caring is hiring a PRN Home Health Physical Therapist to serve La Grange in the Bryan College Station territory. The therapist provides individualized physical therapy in patients’ homes, connecting treatment to each patient’s daily life, abilities, environment, and goals. The position seeks consistent availability, including a commitment to specified days and hours each week. The therapist evaluates patients’ functional needs, develops and delivers treatment plans, tracks progress, adjusts care as needs change, teaches patients and caregivers strategies to support recovery, coordinates with physicians and other healthcare professionals, completes clinical documentation, and supervises therapy assistants and support team members as appropriate.
Requirements
- Graduate from an APTA-approved physical therapy program.
- Hold a current, unrestricted Texas physical therapist license.
- Have at least one year of experience in a healthcare facility.
- Have a valid driver’s license and acceptable driving record, or reliable transportation between patient homes and work locations.
- Be able to frequently lift, push, pull, and support up to 50 pounds, including for patient positioning, transfers, and moving equipment.
- Commit to consistent availability, including the days and hours available to work each week.
Responsibilities
- Evaluate patients’ functional needs and create individualized treatment plans.
- Provide skilled physical therapy in patients’ homes.
- Track patient progress and adjust care as needs change.
- Teach patients and caregivers strategies to support recovery between visits.
- Coordinate care with physicians and other healthcare professionals.
- Complete timely, accurate clinical documentation.
- Supervise therapy assistants and support team members as appropriate.
