Home Health Physical Therapist - PRN
About The Position
CenterWell Home Health is hiring a PRN Home Health Physical Therapist who reports to the Clinical Manager. The therapist provides personalized physical therapy in patients’ homes to help them regain strength, mobility, independence, and functional ability. The role includes assessing patients, planning and administering treatment, coordinating with the physician and care team, and documenting care and progress. The position is patient-facing and requires TB screening. It also participates in Humana’s driver safety program, requiring a valid state driver’s license and personal vehicle liability insurance that meets the stated minimums.
Requirements
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid state driver’s license and personal vehicle liability insurance meeting the applicable requirements.

- One year of experience as a Physical Therapist.
Responsibilities
- Plan and administer prescribed physical therapy treatment for patients in their homes to help them attain the highest possible level of physical function.
- Assess and document patients’ physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement rehabilitation programs consistent with the physician’s Plan of Treatment and the patient team’s goals.
- Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
- Promptly and thoroughly document patient observations, interventions, and evaluations.
- Report patient progress to the physician, Clinical Manager, care team, patient, and family.
- Submit required evaluations, plans, progress notes, and discharge summaries.
