About The Position
Concierge Home Care is seeking a Home Health Physical Therapist (PT) who desires more control over their schedule, meaningful patient relationships, and the ability to increase income based on productivity. This role offers a flexible schedule and autonomy, allowing therapists to create and manage their own schedules. The position emphasizes 1:1 patient care in the home environment, with AI-assisted charting to reduce documentation time and a user-friendly EMR system. Requirements
- Active, unencumbered Florida Physical Therapist license
- Valid driver’s license, reliable transportation, and auto insurance
- Successful completion of an AHCA Level 2 background screening
Responsibilities
- Evaluate patients and develop individualized treatment plans
- Provide therapeutic exercises and mobility training
- Educate patients and caregivers on safety and recovery
- Collaborate with physicians and interdisciplinary teams
- Supervise Physical Therapist Assistants (PTAs)
- Complete documentation using streamlined technology
