About The Position
Concierge Home Care is seeking a Home Health Physical Therapist (PT) to join their team. The coverage area includes Palatka, FL 32177, Putnam County, and surrounding areas, with territories designed to minimize drive time through clustered scheduling. The position emphasizes flexibility, autonomy, and the use of technology to reduce documentation time, allowing therapists to focus more on patient care.
Requirements
- Active, unencumbered Florida Physical Therapist license
- Valid driver’s license, reliable transportation, and auto insurance
Responsibilities
- Evaluate patients and develop individualized treatment plans
- Provide therapeutic exercises and mobility training
- Educate patients and caregivers on safety and recovery
- Collaborate with physicians and interdisciplinary teams
- Supervise Physical Therapist Assistants (PTAs)
- Complete documentation using streamlined technology
