About The Position

Home Care Network, Inc. is seeking a Registered Nurse (RN highly preferred) or Licensed Practical Nurse (LPN w/ several years of experience) to provide Home Health Private Duty Nursing (PDN) services in and around Lancaster, OH (Fairfield County) for a loving young adult male patient with an amazing family (incl. active parental caregivers + additional HCN in-home care team). This is a long-term employment opportunity with a nurse-owned agency that has an excellent reputation. Decisions are made by leaders who have been in the industry for decades, and regional administrators are experienced RNs. The role involves providing skilled nursing functions in accordance with federal, state, and local laws, and within the guidelines of the professional organization and agency. The LPN/RN delivers care to clients as delegated by the RN Case Manager.

Requirements

  • Active Ohio RN or LPN license
  • Minimum of 1 year experience both vent care and tube feedings / G-tube care expertise (2+ years preferred)
  • Minimum of 2 years of experience in paid home health or visiting nurse setting (home health agency experience a plus)
  • Strong medication administration skills
  • Reliable transportation and punctuality
  • Active CPR and First Aid certificates highly desired (for speed to start care), but agency will provide renewal/recertification if required.
  • Good interpersonal communication skills - written and verbal.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful.
  • Ability and willingness to successfully perform all required duties including essential physical demands (listed below).
  • Current driver's license or state ID, plus reliable transportation, are required.
  • Excellent attendance record with past employers.
  • Willingness to consent to a background check, including a criminal record check, and an employment and education verification.

Responsibilities

  • Ventilator & Tracheostomy Management
  • G‑Tube & Enteral Feeding Care
  • Medication Administration
  • Follow physician orders and established Plans of Care
  • Excellent clinical documentation
  • Communicate patient changes to the RN Case Manager + Care Team members
  • Provides professional nursing care under the direction of a Registered Nurse.
  • Communicates appropriate status/condition changes to the RN per agency guidelines.
  • Provides skilled nursing visits for clients as directed by the RN Case Manager.
  • Follows the care plan established by the Registered Nurse.
  • Recognizes and reports changes in client condition to the Case Manager and physician, as directed.
  • Communicates information on a timely basis, consistent with agency policy and urgency of the situation. Communication is verbal, as needed, and through complete and accurate documentation in the clinical record.
  • Scope of clinical practice includes personal care services, assessment of findings, medication monitoring and administration, client teaching, emergency intervention, and nursing services within professional scope of practice and as delegated by the Case Manager.
  • Individuals with the license designation of "LPN M-IV" may perform venipuncture for the purpose of obtaining blood samples for laboratory analysis.
  • Promotes personal safety and a safe environment for clients and coworkers.
  • Demonstrates knowledge of safety/infection control practices by compliance with policies and procedures.
  • Recognizes and responds appropriately to potentially unsafe situations.
  • Demonstrates safe practice in the use of equipment.
  • Makes recommendations on improvement of safety.
  • Performs job in compliance with agency policies and procedures and professional and community standards.
  • Accepts responsibility for professional nursing role.
  • Follows applicable state and federal laws and agency standards.
  • Attends all meetings as required.
  • Maintains confidentiality in all aspects of the job.
  • Refrains from discussing client or staff identities, conditions, or circumstances in public or with others who do not have a legitimate interest in the person.
  • Participates in the development, implementation, and evaluation of the Agency Quality Improvement Program and pertinent activities.
  • Performs other related duties and responsibilities as deemed necessary.
