About The Position
Windsor Home Health, a division of PREMIER THERAPY, is seeking a PRN Occupational Therapist to provide home health care and help patients remain comfortable at home. The company’s mission is to enrich patients’ lives and support independent, dignified living. The role provides individualized, one-on-one occupational therapy in patients’ homes, with a focus on functional abilities, safety, and patient progress.
Requirements
- Graduate of an accredited Occupational Therapy program.
- Current Occupational Therapist license in the state of Ohio.
- Strong communication and organizational skills.
- Valid driver’s license and reliable transportation.
Nice To Haves
- Home health experience is preferred but not required.
Responsibilities
- Evaluate patients’ functional abilities and develop individualized treatment plans.
- Provide occupational therapy services in the home setting.
- Educate patients and caregivers on adaptive techniques and safety strategies.
- Collaborate with physicians, nurses, therapists, and interdisciplinary team members.
- Complete timely and accurate documentation.
- Monitor patient progress and adjust treatment plans as needed.
