Home Health LPN - East Valley
About The Position
Emblem Home Health is hiring a Licensed Practical Nurse (LPN) to provide hands-on patient care in patients’ homes throughout the East Valley, including Mesa, Chandler, and Gilbert. The LPN works under the guidance of a Registered Nurse to support patient comfort, safety, positive outcomes, dignity, and independence. This is a full-time position with a Monday–Friday schedule. Requirements
- Currently licensed as a Licensed Practical Nurse (LPN) in the State of Arizona.
- Current CPR certification.
- Valid driver’s license, auto insurance, and reliable transportation.
Nice To Haves
- Three years of nursing experience preferred; Home Health or MedSurg experience is a plus.
Responsibilities
- Deliver compassionate, direct patient care in accordance with state regulations.
- Implement care plans developed by the Registered Nurse and accurately document patient progress.
- Monitor patient conditions and communicate observations to the Registered Nurse and physician.
- Collaborate with the care team to ensure seamless, high-quality service.
- Participate in professional development and team meetings.
