About The Position

Adoration Home Health is seeking a dedicated Home Health Licensed Practical Nurse to provide patient-centered care in Nashville (Davidson County), Tennessee. The role provides one-to-one care in patients’ homes, following their plans of care while supporting their dignity and independence. The position is full-time and offers flexible scheduling options, independent work with team support, and a $2,500 sign-on bonus. Benefits may vary by employment status.

Requirements

LPN/LPT/LVN degree from an accredited college of nursing.
Current unrestricted registration and license in the state of practice.
Valid driver’s license, acceptable driving record, and proof of car insurance.
Professional CPR certification.

Nice To Haves

One or more years of nursing experience.
Home Health care experience.

Responsibilities

Provide one-to-one care according to each client’s plan of care while maintaining the client’s dignity and independence.
Administer medications, feedings, oxygen, and ostomy care as needed under the client’s plan of care, and assess results.
Accurately document nursing actions, care provided, communications, progress, and outcomes related to established goals.
Inform the physician, case manager, and family of changes in the client’s medical condition and needs.
Coordinate or order supplies, medications, and feedings to ensure adequate amounts are available.
Ensure patient needs are met in accordance with physician instructions.
