Home Health Licensed Practical Nurse
About The Position
As a CenterWell Home Health nurse, the Home Health Licensed Practical Nurse reports to the Clinical Manager and provides personalized, one-on-one skilled nursing care to patients in their homes under the direction of an RN. The role supports patients in regaining independence and recovering at home through individualized care plans developed in collaboration with patients, families, and the healthcare team. The nurse educates patients and families about disease management, treatment options, and self-care; documents visits, interventions, and outcomes; and promptly communicates patient or family needs and changes in status to patients, physicians, clinical managers, and other members of the healthcare team. The position also participates in care coordination and discharge planning.
Requirements
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting.

Responsibilities
