Home Health Licensed Practical Nurse
About The Position
As a CenterWell Home Health nurse, the Home Health Licensed Practical Nurse reports to the Clinical Manager and provides personalized, one-on-one skilled nursing care to patients in their homes under the direction of an RN. The nurse works with physicians, clinicians, patients, and families to implement care plans that support recovery and help patients regain independence. The role includes patient and family education, care documentation, timely communication with the healthcare team, care coordination, and discharge planning. The position is patient-facing and requires participation in Humana's TB screening program. It also falls under Humana's driver safety program, requiring a valid driver's license and personal vehicle liability insurance. Florida-based candidates are subject to Florida Level 2 background screening.
Requirements
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting.

Responsibilities
- Provide skilled nursing care to patients in their homes under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and families about disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient visits, interventions, and outcomes.
- Promptly communicate patient or family needs and changes in status to patients, physicians, clinical managers, and other healthcare team members.
- Participate in care coordination activities and discharge planning.
