About The Position

As a CenterWell Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one-on-one skilled nursing care that helps patients regain independence in their homes. You will work with physicians and clinicians to manage recovery-focused care plans, under the direction of an RN. The role is patient-facing and requires travel to patients' homes using reliable personal transportation. The position is scheduled for 40 hours per week. A $5,000 sign-on bonus is offered.

Requirements

• Current nursing license in the practicing state.
• Valid driver's license, auto insurance, and reliable transportation.
• Current CPR certification.
• Two years of experience as an LPN/LVN in a clinical setting.

Nice To Haves

• Nursing experience in a Home Health or Hospice setting.

Responsibilities

• Provide skilled nursing care to patients in their homes under the direction of an RN.
• Implement individualized care plans in collaboration with patients, families, and the healthcare team.
• Educate patients and families about disease management, treatment options, and self-care techniques.
• Maintain accurate records of patient visits, interventions, and outcomes.
• Promptly communicate patient or family needs and status changes to patients, physicians, clinical managers, and other healthcare team members.
• Participate in care coordination activities and discharge planning.
