Home Health Licensed Practical Nurse
About The Position
As a CenterWell Home Health nurse reporting to the Clinical Manager, you will provide personalized, one-on-one skilled nursing care to patients in their homes under the direction of an RN. You will work with physicians and clinicians to manage individualized care plans that support recovery and help patients regain independence. The role includes educating patients and families about disease management, treatment options, and self-care; maintaining accurate records of visits, interventions, and outcomes; and promptly communicating patient or family needs and status changes to patients, physicians, clinical managers, and other members of the healthcare team. You will also participate in care coordination and discharge planning. The position is patient-facing and requires TB screening. It is covered by Humana’s driver safety program, requiring a valid state driver’s license and appropriate personal vehicle liability insurance.
Requirements
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
