Home Health Licensed Practical Nurse
About The Position
As a CenterWell Home Health nurse reporting to the Clinical Manager, the Home Health Licensed Practical Nurse provides personalized, one-on-one skilled nursing care to patients in their homes under the direction of an RN. The nurse works with physicians, clinicians, patients, and families to support recovery, regain independence, and help patients return to their daily lives. The role includes implementing individualized care plans, educating patients and families, documenting care and outcomes, communicating changes in patient status, and participating in care coordination and discharge planning. This is a patient-facing position subject to Humana's TB screening program and driver safety program. The position is scheduled for 40 hours per week.
Requirements
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting.

Responsibilities
- Provide skilled nursing care to patients in their homes under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and their families on disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient care, including visit documentation, interventions, and outcomes.
- Promptly communicate with patients, physicians, clinical managers, and other healthcare team members regarding patient or family needs and changes in status.
- Participate in care coordination activities and discharge planning.
