High School Graduate
Required computer proficiency
Excellent interpersonal skills

Nice To Haves

2-3 years Home Health intake experience preferred
LVN candidates accepted

Responsibilities

Receives and processes new referrals in a timely manner.
Facilitates care coordination with sales reps, referral sources, physician offices, patients, and agency clinical leaders.
Effectively communicates and relays accurate care coordination details to internal and external constituents in the referral coordination process.
Takes ownership of recording accurate information on the referral tracker and in the EHR and ensuring all referrals move through the process quickly and responds and communicates in a professional manner.
Demonstrates commitment, competency, and ability to handle a lot of details proficiently.
