About The Position

Heritage Home Healthcare is seeking a dedicated and experienced Home Health Field Supervisor to join our leadership team in the Albuquerque, NM area. This role is responsible for overseeing field caregivers and home health aides to ensure the delivery of high-quality, compliant, and client-centered in-home care services. The Field Supervisor plays a critical role in ensuring adherence to care plans, Activities of Daily Living (ADLs), and regulatory standards while supporting caregivers, clients, and clinical leadership in achieving positive care outcomes. This position requires strong leadership, excellent communication, and a solid understanding of home healthcare operations and field staff supervision.

Requirements

  • Minimum of 3 years of experience in home healthcare, caregiving, or a related healthcare field
  • Previous supervisory or leadership experience strongly preferred
  • Strong knowledge of healthcare regulations and best practices preferred
  • High school diploma or equivalent required
  • Strong written and verbal communication skills
  • Strong computer literacy, including Microsoft 365 (Word, Excel, Outlook, Teams)
  • Must have a current valid driver’s license
  • Reliable transportation required
  • Must live in Albuquerque, NM or surrounding areas

Responsibilities

  • Supervise, support, and evaluate field staff including caregivers and home health aides
  • Conduct regular field visits to monitor quality of care and ensure compliance with care plans and agency standards
  • Ensure services are delivered in alignment with Activities of Daily Living (ADLs) and individualized care plans
  • Provide coaching, training, and ongoing development for field staff
  • Participate in caregiver onboarding, orientation, and competency evaluations
  • Address and resolve caregiver and client concerns in a timely and professional manner
  • Collaborate with the Operations Manager and clinical team to improve client care services and outcomes
  • Maintain accurate, timely documentation and ensure compliance with agency policies and procedures
  • Ensure adherence to healthcare regulations, NM DOH standards, and HIPAA requirements
  • Support recruitment efforts and assist with hiring and onboarding new staff
  • Promote safe, ethical, and high-quality client-centered care delivery
