About The Position
As a Home Health RN, you’ll deliver 1:1 care where it matters most—your patient’s home. You’ll have the autonomy to make clinical decisions, build meaningful relationships, and truly see the impact of your care. As a Field RN, you’ll manage your own caseload and provide skilled nursing care across the full patient journey.
Requirements
- Active RN license
- Valid driver’s license & auto insurance
- CPR certification
Nice To Haves
- 2+ years of Home Health experience preferred (strong clinical RNs encouraged to apply)
- Experience with HomeCare HomeBase is a plus
Responsibilities
- Perform start-of-care, recertification, and routine skilled nursing visits
- Conduct comprehensive assessments (physical, psychosocial, environmental)
- Develop and manage individualized Plans of Care
- Educate patients and families on disease management, medications, and self-care
- Identify changes in condition and coordinate care with physicians and the care team
- Supervise and provide direction to LVNs/LPNs and Home Health Aides
- Ensure accurate, timely clinical documentation (HomeCare HomeBase preferred)
- Maintain compliance with Medicare/insurance guidelines
- Participate in on-call rotation
