About The Position

As a Home Health RN, you will have the autonomy and time to provide truly patient-centered care in the comfort of their homes, avoiding rushed shifts and high patient ratios. Requirements

Active RN license
Valid driver’s license, auto insurance & reliable transportation
CPR certification
2+ years of RN experience
Strong clinical judgment and communication skills

Home health experience
Homecare Homebase documentation experience

Responsibilities

Perform start-of-care visits and ongoing patient assessments
Develop and manage individualized care plans
Provide hands-on nursing care, education, and emotional support
Monitor patient progress and adjust care as needed
Collaborate with physicians and interdisciplinary teams
Supervise LVNs/LPNs and Home Health Aides
Ensure accurate, timely documentation (Homecare Homebase preferred)
Build strong relationships with patients and families
