Requirements

• Active and unrestricted Registered Nurse (RN) license
• Experience in home health, med-surg, or acute care preferred
• Strong assessment, leadership, and documentation skills
• Ability to supervise and delegate to LPNs/LVNs and care staff
• Compassionate communicator with a patient-first mindset
• Ability to work independently and manage multiple cases in the field
• Current RN License
• Valid Driver’s License
• CPR Certification
• Valid Auto Insurance

Nice To Haves

• At least 2 years of Home Health RN experience preferred
• HomeCare HomeBase experience preferred

Responsibilities

• Performing comprehensive patient assessments (physical, psychosocial, functional, and environmental)
• Developing, implementing, and updating personalized plans of care
• Providing skilled nursing care for patients with complex and chronic conditions
• Educating patients and families on self-care, medications, nutrition, and disease management
• Evaluating patient progress and adjusting care plans as needed
• Communicating changes in patient status to physicians and interdisciplinary team members
• Ensuring strict adherence to infection control and safety standards
• Supervising and directing LPNs/LVNs and Home Health Aides to ensure safe, effective, and high-quality care delivery
• Monitoring clinical documentation to ensure compliance with regulatory and payor requirements
• Reviewing patient outcomes and ensuring continuity of care across the team
