Are you an RN who wants to practice at the top of your license and truly make a difference? Join our Home Health team where you’ll have the flexibility, independence, and support to deliver exceptional patient care—right where it matters most.

As a Home Health RN, you’ll take the lead in managing patient care from start to finish. You’ll complete OASIS assessments, develop personalized care plans, and oversee LPNs to ensure the highest standards of care are delivered in the home.

Requirements

• Current RN License
• Valid Driver’s License
• CPR Certification
• Valid Auto Insurance

Nice To Haves

• At least 2 years of Home Health RN experience preferred
• HomeCare HomeBase experience preferred

Responsibilities

• Complete OASIS assessments and manage documentation
• Perform comprehensive patient evaluations
• Develop and adjust individualized care plans
• Supervise and support LPNs in the field
• Collaborate with physicians and interdisciplinary teams
• Educate patients and families for better outcomes
