If you’re a nurse who values autonomy, critical thinking, and personalized care—this role is for you. ---------------------------------------- 💙 WHAT YOU’LL DO As a Home Health RN, you will serve as the clinical leader in the home setting, responsible for: 🩺 Performing comprehensive patient assessments (physical, psychosocial, functional, and environmental) 📋 Developing, implementing, and updating personalized plans of care 🏡 Providing skilled nursing care for patients with complex and chronic conditions 👨👩👧 Educating patients and families on self-care, medications, nutrition, and disease management 🔄 Evaluating patient progress and adjusting care plans as needed 📞 Communicating changes in patient status to physicians and interdisciplinary team members 🦠 Ensuring strict adherence to infection control and safety standards 🧑⚕️ Supervising and directing LPNs/LVNs and Home Health Aides to ensure safe, effective, and high-quality care delivery 📊 Monitoring clinical documentation to ensure compliance with regulatory and payor requirements 📈 Reviewing patient outcomes and ensuring continuity of care across the team ---------------------------------------- 🌈 WHY THIS ROLE STANDS OUT Provide 1:1 patient care in a comfortable home setting Lead and support a clinical care team, including LPNs/LVNs Enjoy independence and clinical variety in your daily work Make a direct impact on patient independence and quality of life Collaborate with a supportive interdisciplinary team See meaningful, real-world patient progress every day ---------------------------------------- 💡 WHAT WE’RE LOOKING FOR Active and unrestricted Registered Nurse (RN) license Experience in home health, med-surg, or acute care preferred Strong assessment, leadership, and documentation skills Ability to supervise and delegate to LPNs/LVNs and care staff Compassionate communicator with a patient-first mindset Ability to work independently and manage multiple cases in the field
Requirements
- Active and unrestricted Registered Nurse (RN) license
- Strong assessment, leadership, and documentation skills

- Current RN License
- Valid Driver’s License
- CPR Certification
- Valid Auto Insurance
Nice To Haves
- Experience in home health, med-surg, or acute care preferred
- At least 2 years of Home Health RN experience preferred
- HomeCare HomeBase experience preferred
