About The Position
Join a growing home health team as a compassionate, skilled Home Health RN, delivering care where patients feel safest: in their homes. This role offers independence, meaningful patient relationships, and flexibility without sacrificing clinical excellence. You will provide one-on-one patient care, manage diverse clinical cases from chronic care to post-acute recovery, and benefit from a strong interdisciplinary support team. This is a relationship-based nursing role, distinct from a hospital floor position, offering the freedom of field nursing with collaborative clinical team support.
Requirements
- Active Registered Nurse (RN) license
- Strong clinical judgment and assessment skills
- Ability to work independently in the field
- Excellent communication and patient education skills
- Valid driver’s license, reliable transportation, and auto insurance
- Current CPR certification
Nice To Haves
- Minimum of 2 years of nursing experience (Home Health preferred)
- Experience with HomeCare HomeBase preferred (not required)
Responsibilities
- Conduct comprehensive in-home patient assessments
- Develop and update individualized care plans
- Manage medications, wound care, and chronic disease support
- Educate patients and families on self-care and health management
- Coordinate care with physicians and interdisciplinary teams
- Supervise and support LPNs and Home Health Aides
- Ensure accurate, timely documentation of patient progress
- Monitor and adjust care based on patient condition changes
