About The Position

Puget Sound Home Health & Hospice, a provider of exceptional care for over 30 years, is expanding and seeking a dedicated Home Health Clinical Manager for its Pierce County division. This role is crucial for ensuring compliance with home health and hospice laws and regulations, overseeing patient care, and supporting the clinical team. The ideal candidate will be a licensed RN, PT, or OT with experience in a Medicare-certified home health agency and OASIS training. Requirements

• Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT) license
• Experience in a Medicare-certified home health agency
• OASIS trained
• Minimum 2 years of healthcare management experience
• At least 1 year of home health/hospice experience with OASIS training
• Current BLS certification
• Washington state license
• Knowledge of Medicare COPs, PDGM, VBP
• Licensed and insured driver

Nice To Haves

• BSN degree preferred
• Experience with Home Care Home Base preferred

Responsibilities

• Ensure compliance with home health/hospice laws and regulations
• Review referrals and manage assignments
• Oversee patient charts and clinical courses
• Assist with staff training, hiring, and mentoring
• Participate in QAPI, OASIS review, and coding workflow
• Some direct patient care and on-call duties required
