About The Position

JOIN THE GROWING TEAM AT CHOICE HEALTH AT HOME AND HELP EXPAND ACCESS TO COMPASSIONATE, HIGH-QUALITY HOME HEALTH SERVICES THROUGHOUT THE COMMUNITY. WE ARE SEEKING A RELATIONSHIP-DRIVEN, RESULTS-ORIENTED PROFESSIONAL WITH RECENT HOME HEALTH OR HOSPICE SALES EXPERIENCE WHO THRIVES ON BUILDING REFERRAL PARTNERSHIPS AND DRIVING CENSUS GROWTH. In this role, you will serve as a key ambassador for our organization by developing strong relationships with physicians, hospitals, case managers, skilled nursing facilities, assisted living communities, and other healthcare partners. You will collaborate closely with clinical and operational leaders to ensure patients receive seamless, high-quality care while helping Choice remain the preferred provider in the markets we serve.

Requirements

  • Minimum of 2 years of recent home health or hospice sales experience preferred
  • Proven ability to develop referral relationships and grow market share
  • Strong communication, presentation, and relationship-building skills
  • CRM software experience preferred
  • HomeCare HomeBase experience preferred
  • Valid driver’s license and current auto insurance required
  • Self-motivated, goal-oriented, and highly organized professional

Responsibilities

  • Establish and grow referral relationships, positioning Choice as the home health provider of choice within the community
  • Develop strategic partnerships with physicians, hospitals, case managers, senior living communities, and healthcare organizations
  • Promote Choice’s patient-centered care model through proactive outreach and relationship management
  • Identify new business opportunities and support market expansion initiatives
  • Provide insight regarding community needs, program development, staffing needs, and growth opportunities
  • Collaborate with leadership to develop and maintain policies related to new programs and services
  • Maintain compliance with all local, state, and federal regulations related to healthcare marketing
  • Partner with the interdisciplinary team to support safe, effective, and compassionate patient care
  • Lead community outreach and public relations efforts to increase awareness of agency services
  • Meet and exceed monthly admission and growth goals
  • Utilize CRM tools and reporting systems to manage referral activity and pipeline growth
