About The Position

Are you a proven home health sales professional who knows how to build referral relationships, grow admissions, and become a trusted resource in the healthcare community? Join Choice Health at Home as a Home Health Business Development Liaison and help expand access to exceptional patient-centered care. In this role, you’ll serve as the face of our agency within the medical community — partnering with physicians, hospitals, case managers, rehab facilities, and community organizations to grow referrals and strengthen our reputation as the provider of choice. We’re looking for someone who thrives on relationship-building, understands the home health industry, and has a track record of exceeding goals.

Requirements

  • Recent Home Health or Hospice sales experience required
  • Proven success building and maintaining referral relationships
  • Strong understanding of healthcare sales, territory management, and account development
  • Valid Driver’s License and current auto insurance required

  • HomeCare HomeBase experience preferred
  • Excellent communication, presentation, and relationship-building skills

Responsibilities

  • Develop and grow referral relationships with physicians, hospitals, SNFs, assisted living facilities, and community partners
  • Promote Choice Health at Home’s services and clinical excellence throughout the market
  • Execute strategic outreach plans to increase admissions and market presence
  • Collaborate with clinical and operational leadership to support patient care and referral success
  • Identify community needs and provide recommendations for program growth and development
  • Maintain knowledge of all federal, state, and local healthcare marketing regulations
  • Partner with interdisciplinary teams to ensure seamless patient transitions and exceptional service
  • Represent the agency through networking events, community outreach, and public relations initiatives
  • Meet and exceed monthly admission and growth goals
