About The Position

Choice Health At Home is seeking a driven, relationship-focused professional to serve as a Home Health Business Development Liaison. This role involves connecting patients and families with home health services by networking, conducting community outreach, and building referral partnerships. The liaison will be the community face of the organization, developing relationships with physicians, hospitals, case managers, senior living communities, and other referral partners. The goal is to help patients receive quality home care and contribute to the organization's growth.

Requirements

  • 2+ years of home health or hospice sales experience preferred
  • Strong relationship-building and communication skills
  • Experience with CRM software preferred
  • HomeCare HomeBase experience preferred
  • Valid Driver’s License and Auto Insurance

Responsibilities

  • Build and strengthen referral relationships to position Choice Health At Home as the provider of choice
  • Develop new business opportunities through strategic community outreach and networking
  • Collaborate with clinical and operational teams to ensure smooth patient transitions and exceptional care experiences
  • Promote awareness of Choice Health At Home services within the healthcare community
  • Meet and exceed monthly referral and admission goals
  • Maintain knowledge of industry regulations and ensure compliant marketing practices
  • Help identify community needs and contribute to program growth initiatives
