About The Position

At Choice Home Health, we believe exceptional patient care begins with strong community partnerships. We're seeking an energetic, relationship-driven Home Health Business Development Liaison to help expand our presence by building lasting referral relationships throughout the communities we serve. If you're a healthcare sales professional who enjoys connecting with physicians, hospitals, case managers, and senior care providers—and you're motivated by helping patients receive the care they need—we'd love to meet you. As our Business Development Liaison, you'll serve as the face of Choice Home Health, developing strategic partnerships that improve access to quality home health services while driving agency growth.

Requirements

  • Valid driver's license, reliable transportation, and current auto insurance
  • Self-starter with excellent organizational skills and a results-driven mindset

Nice To Haves

  • 2+ years of recent home health or hospice business development/sales experience
  • Proven success growing referral networks and increasing patient admissions
  • Strong relationship-building, networking, presentation, and communication skills
  • Experience with CRM platforms preferred
  • Homecare Homebase experience is a plus

Responsibilities

  • Build and strengthen referral relationships with physicians, hospitals, case managers, skilled nursing facilities, assisted living communities, and other healthcare partners
  • Position Choice as the preferred home health provider through exceptional customer service and relationship management
  • Identify new referral opportunities and execute strategies to grow market share
  • Promote Choice's patient-centered care model through community outreach, networking, and educational presentations
  • Collaborate with clinical and operational leaders to ensure seamless transitions of care and exceptional patient experiences
  • Analyze market trends and identify opportunities for service expansion and business development
  • Represent Choice at community events, healthcare meetings, and networking opportunities
  • Maintain compliance with all federal, state, and organizational healthcare marketing regulations
  • Track referral activity, sales pipeline, and growth initiatives using CRM tools
  • Meet and exceed monthly admissions, referral, and growth goals
