About The Position

VitalCaring is seeking a compassionate, dependable Home Health Aide (HHA) to provide hands-on support, comfort, and dignity to patients in their homes. The role focuses on helping patients maintain comfort, safety, and independence while building meaningful relationships with patients and families. The HHA works independently in a home-based setting and collaborates with nurses, therapists, and other care team members.

Requirements

• Meet state and federal requirements for Home Health Aide certification or training.
• Be at least 18 years of age.
• Ability to read, follow care plans, and document services provided.
• Current CPR certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Ability to work independently with strong time management skills.
• Demonstrate empathy, patience, and professionalism in all interactions.

Responsibilities

• Assist patients with daily living activities, including bathing, dressing, grooming, and toileting.
• Support mobility, transfers, and basic exercises in alignment with the care plan.
• Help maintain a clean, safe, and comfortable home environment.
• Prepare meals and assist with feeding as needed.
• Take and record vital signs when required and trained to do so.
• Assist with medication self-administration as directed.
• Observe and report changes in patient condition to the clinical team.
• Document care provided accurately and timely within the EMR system.
• Participate in team meetings, trainings, and care coordination activities.
• Provide compassionate support to patients and families during each visit.
