About The Position

The Home Health Aide is a Certified Nurse Assistant that provides direct personal and supportive care for home care patients on an intermittent or hourly basis, based on physician orders. The Home Health Aide provides patient care and follows tasks as assigned on the patient’s individual plan of care, designed by the Registered Nurse. The Home Health Aide reports directly to the Clinical Staff Lead. This position works in conjunction with other members of nursing service, ancillary services, physicians, patients, and families. It collaborates with various members of the healthcare team to ensure quality of care across the continuum. Requires the ability to be self-directed and quality focused. Interpersonal communication skills are a necessary function of this position.

  • Planning & Organizing (Fundamental Awareness): Demonstrates ability to anticipate basic resource needs, identify discrete tasks, set priorities, define dependencies, schedule activities, meet deadlines, and organize work for own assignments and responsibilities. Appropriately matches equipment, and capital resources to task demands. Prepares and presents clear and accurate status reports on projects and assignments.
  • CLINICAL COMPETENCIES HOME CARE (NOVICE): Demonstrates a basic proficiency to assist and perform direct personal care within the scope of the certification.
  • DOCUMENTATION IN EMR (FUNDAMENTAL AWARENESS): Has experience or training working with clinical applications for patient documentation.
  • ENVIRONMENT OF CARE HOME CARE(FUNDAMENTAL AWARENESS): Has limited experience or training working with set up and maintenance of patient’s home environment, supplies and equipment.

Responsibilities

  • Reviews and performs tasks as defined on the personal care plan of the individual patient.
  • Responsibilities vary based on the care plan but may include direct assistance with activities of daily living, bathing, toileting, obtaining vital signs, intake and outputs, weights, transferring, provides facial shaving, oral care, hair care and dressing.
  • The Home Health Aide reports any abnormal findings to the nurse and/or CSL.
  • The Home Health Aide communicates with the clinical team in a timely manner.
  • The Home Health Aide works under the supervision of the Registered Nurse or Qualifying therapist.
  • The Home Health Aide adheres to practices and policies outlined to ensure patient safety and follows infection control standards.
