About The Position

Interim HealthCare is seeking a compassionate and dependable Home Health Aide (CNA) to provide personalized, patient-centered care in patients’ homes. The role supports patients’ comfort, independence, safety, and quality of life by assisting with daily living activities, monitoring well-being, and following individual care plans. The position provides dedicated 1:1 patient care and works in coordination with home health nurses and aides.

Requirements

• High school diploma or equivalent.
• Active Nursing Assistant Certification (CNA) in South Carolina.
• Valid driver’s license, auto insurance, and reliable transportation.
• Compassionate nature and good communication skills.
• Ability to lift up to 50 lbs.

Nice To Haves

• Home Health experience preferred.

Responsibilities

• Care for seniors and people with an illness, injury, or chronic disease in their homes.
• Assist with daily activities, including bathing, toileting, dressing, grooming, hygiene, nail and skin care, eating, and nutritional intake.
• Assist with ambulation, transfers, and range-of-motion exercises.
• Check vital signs, measure intake and output, monitor blood glucose, and document changes.
• Work with home health nurses and aides to ensure the patient’s care plan is followed.
• Observe patients and notify a supervisor of issues that may require a change in care, medication, or medical equipment.
