About The Position

This role is ideal for a dedicated HHA who values flexibility, with a schedule listed below: 12 hour shift available (It varies on the schedule available) Must be available to work in Lorain County (Elyria) It can be a 7a-7p | 7p-7a| 8a-8p |8p-8a or other schedule available.

Requirements

High school diploma or GED
Minimum one (1) year of documented home care assistance and/or direct care experience at an assisted living or skilled nursing facility, etc., is preferred
Successful completion of a nursing assistant or certified home health aide program
Certified CPR and negative TB skin test or chest x-ray
Must have a valid driver's license and auto insurance (proof of insurance is required)
Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull. Varied assistance is required depending on the mobility of the client.
Must be able to lift up to 50 pounds
Self-directing with the ability to work with little direct supervision
Ability to adhere to HIPAA and ensure confidentiality and security of the client's medical information
Ability to read, write, speak, and understand English and communicate effectively
Up-to-date vaccinations including COVID-19, Influenza, and Hepatitis B

Responsibilities

Provide client-facing care as assigned
Provide assistance with daily tasks as directed in the client's personalized plan of care, including but not limited to: feeding, meal preparation, linen changes, light housekeeping, providing transportation to doctor appointments, outdoor activities, and shopping
Provide companionship, personal and hygiene care as directed in the client's personalized plan of care, including but not limited to: bathing, grooming, oral care, dressing, undressing, toileting activities, and assisting with measurements, if necessary
Adhere to the policies, procedures, and protocols of the agency of employment
Adhere to HIPAA laws and ensure confidentiality and security of the client's medical information
Assist with basic Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene
Needs help in the morning, dressed, maybe a shower, and meal prep.
ADLs Assistance with all ADLs, assist with mobility/transfer; meal prep, medication reminders
Provide light housekeeping to maintain a clean and safe home environment
Run errands and accompany the client to appointments, including barber visits as needed
Provide companionship and support during daily activities to ensure the client's comfort and well-being
Monitor and report any changes in the client's condition to supervisors or case managers
Follow the care plan and communicate regularly with the care team
Stay Ready: Uniform & ID on, phone charged, childcare arranged, and transportation available
