About The Position

Grace At Work is seeking a Home Health Aide to provide one-on-one care to patients in their homes according to individual care plans. The role involves providing personalized and attentive care, building genuine relationships with patients, and helping maintain a healthy environment that meets their needs. The aide should be detail-oriented, highly organized, and have strong verbal and written communication skills. The posting notes that prior Home Health or Hospice experience is a plus. Requirements

Graduation from an accredited Home Health Aide program.
High School Diploma or GED.
One year of prior professional experience.
Driver’s license required.
CPR certification required.
Strong verbal and written communication skills.
Detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs.

Prior Home Health or Hospice experience.

Responsibilities

Assist patients with daily activities, including moving in and out of beds, baths, wheelchairs, or automobiles.
Change bed linens, do laundry, clean the home, and assist with personal care.
Maintain records of patient care, condition, progress, or problems, and report and discuss observations with the supervisor.
Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert.
Give medications and immunizations.
Engage patients in exercises or other activities.
