About The Position

Horizon Home Healthcare LLC is seeking Home Health Aides to provide in-home care to clients in Montgomery County, Ohio, and surrounding areas. The role supports clients in remaining safe, comfortable, and independent in their homes. The position is offered as full-time, part-time, or PRN work. Caregivers assist clients with activities of daily living and mobility, prepare light meals, complete care-related household tasks, monitor and report changes in client condition, provide companionship, and document care. Candidates must meet one of the listed certification or experience requirements and have a valid driver’s license, reliable transportation, and proof of auto insurance.

Requirements

Meet one of the following: hold a current active Ohio State Tested Nursing Assistant (STNA) license; hold an Ohio Certified Nursing Assistant (CNA) license issued less than two years ago; or have one year of verifiable experience within the last two years under the supervision of an RN.
Have a valid driver’s license, reliable transportation, and proof of auto insurance.
Be able to work independently while following a personalized care plan.
Have excellent communication and interpersonal skills.

Nice To Haves

Previous caregiving or home health experience is preferred but not required.
First aid certification is preferred but not required.

Responsibilities

Assist clients with activities of daily living, including bathing, dressing, grooming, and toileting.
Help clients with safe transfers, ambulation, and mobility.
Prepare light meals and assist with feeding as necessary.
Perform light housekeeping, laundry, and other household tasks related to the client’s care.
Monitor and report changes in the client’s condition to the supervising nurse and care team.
Provide companionship and emotional support.
Maintain accurate documentation of care provided.
