About The Position

This position provides companionship and personal care to clients, maintaining records of personal health care activities as defined by regulatory environments. The Home Health Aide (CNA) will adhere to the agency's dress code, present a professional appearance, and maintain a positive demeanor. Responsibilities include assisting clients with personal hygiene, ambulation, eating, dressing, and other assigned duties. The role also involves maintaining a clean and safe environment, performing specific health-related tasks as taught by licensed professionals, and observing clients for any changes in appearance or behavior, reporting these to the registered nurse. Supervision of self-administered medication is limited to specific supportive actions. Personal care activities encompass hygiene, ambulation support, meal preparation and feeding, dressing assistance, toileting support (including catheter/colostomy bag management), bathing assistance, and transfer assistance. The position may also involve other related duties as required or assigned.

Requirements

  • High school or GED, plus specialized schooling and/or on the job education in a specific skill area.
  • 12 to 18 months related experience and/or training, or equivalent combination of education and experience.
  • Visual/hearing acuity sufficient to understand written/oral communication and to observe the client's condition.
  • Good body mechanics.
  • Must be able to push approximately 250 pounds in a wheelchair.
  • Must be able to lift between approximately 50-100 pounds.
  • Must be able to work under stress and cope with sometimes difficult, confused, disoriented and physical disabled clients.
  • Familiarity with and use of proper lifting techniques.
  • Compliance with the Agency safety program.
  • Use of safety equipment if issued.
  • Regularly required to stand, walk, sit, talk, hear.
  • Occasionally required to reach with hands and arms, stoop, kneel, and crouch.
  • Must have excellent balance.
  • Must be able to work at heights.
  • Specific vision abilities required include close, distance, and peripheral vision, depth perception and the ability to adjust focus.
  • Must be able to communicate verbally with others on the phone or in person.
  • Follows the direction of the nursing care plan under the guidance of the Director of Home Health/Designee.
  • Accepts responsibility for completing assignment(s) once it has been accepted.

  • May be required to take a client to physician's appointment, shopping, or errands.
  • If transportation is involved, must have a current driver's license, require auto inspection and carry adequate auto insurance.

Responsibilities

  • Perform personal care activities including assisting with personal hygiene, ambulation, eating, dressing, shaving, physical transfer, and other duties as assigned.
  • Maintain a clean, safe, and healthy environment for the client, including light cleaning and straightening of living areas and laundry.
  • Assist with specific health-related tasks as taught by a licensed health professional, such as changing colostomy bags, assisting with assistive devices, range of motion exercises, simple urine tests, measuring special diets, measuring fluid intake/output, and measuring vital signs.
  • Keep records of personal health care activities.
  • Observe appearance and gross behavioral changes in the client and report to the registered nurse.
  • Supervise self-administered medication by obtaining the container, ensuring it is prescribed, reminding the client of the time, and observing self-administration.
  • Assist with personal hygiene/grooming, ambulation, eating/meal preparation, dressing, toileting, bathing, and transfers.
  • Perform any other related duties as required or assigned.
