About The Position
Metro Physical & Aquatic Therapy is seeking a motivated and passionate per diem Physical Therapist to provide in-home care in Eastern Suffolk County. Candidates at all experience levels are encouraged to apply, and the company will train the right new graduate. The role offers flexible scheduling and autonomy, with the ability to see patients any seven days of the week and a preference for assignments in the therapist’s primary geographic area. The position offers 1099 and W-2 work options. The anticipated pay range is $70–$90 per visit; compensation depends on factors including relevant experience, knowledge, skills, other job-related qualifications, and geography.
Requirements
- Graduate of a Physical Therapist program.
- Licensed Physical Therapist in New York State or eligible for licensure.
- Ability to travel to patients’ homes within the assigned geographic territory.
Nice To Haves
- Home health experience is preferred but not required.
Responsibilities
- Accept and treat patients within the designated geographic area.
- Evaluate patients’ physical therapy needs in the home and develop individualized treatment plans.
- Develop plans of care and treatment goals in consultation with physicians, patients, and families.
- Plan, prepare, and implement customized therapy programs to restore function and promote independence.
- Instruct family members, PTAs, home health aides, and caregivers in appropriate therapy techniques.
- Provide timely and accurate documentation for each visit.
- Communicate effectively with patients, families, and interdisciplinary team members, especially regarding discharge planning.
- Deliver care in compliance with agency policies and professional practice standards.
