About The Position

Metro Physical & Aquatic Therapy is seeking a motivated and passionate per diem Physical Therapist to provide in-home care in Glen Cove and neighboring towns. The role involves evaluating patients in their homes, developing individualized plans of care, and delivering therapy to restore function and promote independence. Applicants at all experience levels are encouraged, and the organization will train the right new graduate. Home health experience is preferred but not required; a mentor will be provided to help with documentation and compliance. The position offers flexible scheduling and autonomy, with the ability to see patients any seven days of the week and a preference for serving patients in the therapist’s primary geographic area. Pay is listed as $70–$90 per visit and also as $70–$90 hourly.

Requirements

Graduate of a Physical Therapist program.
Licensed Physical Therapist in New York or eligible for licensure.
First Aid, AED, and CPR certifications are mandatory.
Ability to travel to patients’ homes within the assigned geographic territory.

Nice To Haves

Home health experience is preferred but not required.

Responsibilities

Accept and treat patients within the designated geographic area.
Evaluate patients’ physical therapy needs in the home setting and develop individualized treatment plans.
Develop plans of care and treatment goals in consultation with the physician, patient, and family.
Plan, prepare, and implement customized therapy programs to restore function and promote independence.
Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
Provide timely and accurate documentation for each visit.
Communicate effectively with patients, families, and interdisciplinary team members, especially regarding discharge planning.
Deliver care in compliance with agency policies and professional practice standards.
