The Homecare Coordinator is a core member of the PACE Interdisciplinary Team responsible for coordinating, overseeing, and monitoring homecare services delivered by internal staff and contracted vendor agencies. This role ensures that authorized homecare services are delivered in accordance with participant care plans, regulatory expectations, and organizational policies. The Homecare Coordinator actively participates in IDT care planning, assessment, and reassessment processes; monitors service delivery and aide performance; identifies care gaps or changes in participant status; and facilitates timely care plan modifications.

Requirements

  • CNA, HHA, or equivalent experience required, LPN or supervision experience strongly preferred or other related experience working within a care team model.
  • Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
  • Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
  • Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.

Nice To Haves

  • Experience in mentorship, lead roles, or team coordination (strongly preferred)
  • Experience providing personal care to frail elderly adults (strongly preferred)
  • 1–3 years homecare or PACE experience preferred.

Responsibilities

  • Serves as an active and consistent member of the PACE Interdisciplinary Team.
  • Participates in initial, annual, semi-annual, unscheduled, and Significant Change in Condition (SDR) assessments.
  • Collaborates with nursing, therapy, social work, and providers to develop and update care plans.
  • Identifies changes in participant status and initiates IDT reassessment discussions.
  • Coordinates authorized homecare services in alignment with approved care plans.
  • Oversees internal and vendor-provided aide services.
  • Monitors timeliness, consistency, and adherence to care plan tasks.
  • Serves as the primary point of contact for scheduling and coverage issues.
  • Provides direct care in home to support participant needs when necessary.
  • Serves as assist for in-center activities or participant care if delegated by Center Leadership.
  • Ensures accurate and timely completion of aide documentation.
  • Reviews records for consistency with authorized services.
  • Communicates missed care or refusals to the IDT.
  • Creates and updates homecare authorizations as needed.
  • Coordinates onboarding and orientation for internal aides and resource for vendor.
  • Provides ongoing coaching and support for complex cases.
  • Identifies education gaps and facilitates training.
  • Serves as liaison between the PACE program and contracted agencies.
  • Communicates care plans, schedules, and expectations.
  • Monitors vendor performance and escalates concerns.
  • Monitors and reports incidents, grievances, and aide change requests.
  • Participates in audits and quality improvement activities.
  • Supports participant satisfaction follow-up.
