As a Care Coordinator, you'll serve as the connection between our clients, families, caregivers, and nurses, helping ensure the right care is delivered at the right time.

Requirements

  • High School Diploma or equivalent required
  • Strong organizational and computer skills

Nice To Haves

  • Previous office, scheduling, staffing, or healthcare experience preferred
  • Experience working with elderly, disabled, or medically fragile individuals preferred
  • Strong communication, customer service, and problem-solving skills
  • Ability to multitask and prioritize in a fast-moving environment
  • Professional, positive, and team-oriented attitude

Responsibilities

  • Coordinate daily schedules for caregivers and nursing staff
  • Build and maintain strong relationships with clients and families
  • Communicate schedule changes and care updates promptly
  • Support and assist field staff with day-to-day needs
  • Investigate and resolve client and employee concerns
  • Maintain accurate documentation and reporting
  • Partner with clinical and operational teams to ensure quality care
  • Assist with ongoing quality improvement initiatives
